You can shop longer leaning on the cart than walking upright. Standing tightens the legs; sitting releases them. That 'shopping-cart sign' is the signature of lumbar spinal stenosis — a narrowing that crowds the nerves — and while care can't widen bone, it can often meaningfully change how far your legs will take you.

What’s actually going on

Stenosis is narrowing of the spinal canal or nerve doorways, usually from age-related change: thickened ligaments, bulging discs, arthritic joints each donating a millimeter. Symptoms typically appear with upright posture and walking (heaviness, cramping or tingling in the legs) and ease with sitting or leaning forward — flexion literally makes more room.

Common causes we see

  • Age-related degenerative change — the common path
  • Disc bulges narrowing an already-snug canal
  • Facet joint arthritis and ligament thickening
  • Congenitally narrow canals that hit symptoms earlier

How we evaluate it

History does heavy lifting here (the posture-dependent pattern is distinctive), plus neurological examination, gait observation, and vascular screening — poor circulation mimics stenosis and needs different care entirely. Imaging is coordinated when the picture warrants confirmation or surgical consult looms.

How chiropractic care may help

Honest framing: conservative care doesn't remove bone or ligament — it maximizes the room and fitness you have. Flexion-biased care (positions and exercises that open the canal), joint mobilization for the segments and hips that still move, walking-tolerance programs (bike-first strategies work because cycling flexes the spine), and modalities for flare-ups. Many stenosis patients meaningfully extend their walking distance with structured conservative care; when symptoms progress despite it, we coordinate the surgical conversation honestly.
