Good Faith Estimate — Your Right to Know Costs
Your No Surprises Act right to a Good Faith Estimate at Chiropractic Health Center — who qualifies, what it includes, delivery timelines, and the $400 dispute process, in plain English.
Your federal right
Effective date: July 3, 2026. Under the federal No Surprises Act, health care providers — including chiropractic physicians — must give patients who are uninsured or choosing not to use insurance (self-pay) a written Good Faith Estimate (GFE) of expected charges before scheduled care, and on request. This page explains that right in plain English and how it works at Chiropractic Health Center.
Who receives a Good Faith Estimate
- You have no insurance; or
- You have insurance but choose not to bill it for this care (self-pay).
- Insured patients billing their plan don't receive a GFE for that care — instead, we verify your benefits before your first visit and explain your expected copays or coinsurance up front, and your plan sends an Explanation of Benefits after claims. Different mechanism, same goal: no surprises.
What the estimate includes
Your GFE lists the items and services reasonably expected for your care at this office — for example the initial examination, adjustments, and physiotherapy services if recommended — each with its expected charge, along with our name, license and location. If your care plan changes after examination findings, you're told before additional care begins and may request an updated estimate. A GFE is an estimate, not a bill and not a contract; it doesn't obligate you to receive any service.
When you receive it
- Care scheduled at least 3 business days ahead — the estimate is provided within 1 business day of scheduling.
- Care scheduled at least 10 business days ahead — within 3 business days of scheduling.
- On request, any time, without scheduling anything — within 3 business days. Asking for an estimate never commits you to care.
If the bill exceeds the estimate by $400 or more
Keep your Good Faith Estimate. If you're later billed at least $400 more than the estimate for that provider, you may use the federal patient–provider dispute resolution process: you start it within 120 calendar days of the bill date, a small administrative fee applies, and an independent reviewer decides the fair amount. Our honest expectation: you'll never need that paragraph here. Fees are discussed before care, benefits are verified first for insured patients, and surprise billing is the exact opposite of how this office has kept patients for three decades — but the right is yours and you should know it.
