Can neck adjustments cause strokes?

The short answer
Here's the honest answer most websites dodge: serious events after neck manipulation are extremely rare, and the best-known population study (Cassidy 2008, Spine) found stroke association was similar after chiropractic visits and after regular doctor visits — suggesting patients with an artery problem already in progress seek care for the neck pain it causes. Causation is considered unlikely; screening still matters, and we do it.
The full picture
The concern involves the vertebral arteries, and the Cassidy study's insight was elegant: if manipulation caused these strokes, chiropractic visits should carry more risk than GP visits — they didn't. People with a dissection-in-progress get neck pain and headaches, and they take those symptoms to whichever provider they use.
What that means practically: the screening burden is real for every provider who sees neck pain. Our new-patient examination includes the history and risk-factor screening that flags suspicious presentations for medical evaluation instead of adjustment — sudden severe unfamiliar headache, certain patterns after trauma, and other findings we take seriously.
